Shared Service

Remote Smart Health can get your Doctor connected with one patient or one thousand patients. Shared Services can be used for a temporary start-up or as the long term solution.

Benefits of Shared Services………

The doctor selects the patients from their practice whom they want to be apart of the RPM program. The first piece of equipment will be sent out to the patient based on the patient’s chronic condition. A new product will be shipped every 90 days until the full suite of products is delivered to the patient’s home. The doctor will not be invoiced any charges for the equipment or the services provided through your company and supplied by 101. 101 will bill the CPT Code 99454 directly to Medicare and be reimbursed prominently $69.00. The doctor will bill Medicare the other related codes that will average $101.00 per month.

Welcome Call

The same day that the patient signs up with RPM services from your doctor the personalized care begins. Starting with a Welcome Call and then 101 patient support center will continue to hold the patient’s hand as tightly as needed or wanted to keep the patient engaged and active in their health.

Product Training

Every 90 days a new piece of Home monitoring equipment will be shipped to the patient’s home with a followup call to confirm delivery and to confirm that the patient is comfortable with the use of the equipment. The patient support center will monitor that the patient is testing as prescribed by the doctor.

Patient Engagement

101 primary goal is to engage the patient with their health. Gently pushing them down the path of taking their test as prescribed and visually seeing the outcomes of their lifestyle. What happens with minor adjustment and how to recognized dangerous trends.
